PADI Open Water Diver certification

A student diver must present the PADI Universal Referral Form for Open Water Diver Certification to a dive instructor. It states that a diver had initial training from another diving organization. The form directs the diving instructor to complete the student's open water dives, sign a statement, and send the form to the student's original instructor, who should certify the diver.

You can also use the Universal referral form for certification. However, PADI requires you to complete the course within 12 months of your last training. This usually takes two to three days. You must be able and willing to dive with your buddy up to 60 feet/18 metres.

Statement for medical purposes

To get certified as a diver, you need to complete the Medical Statement to Universal Referral Form. This form is essential to make sure that you are as healthy and fit as possible before you start diving. It's designed to ensure your safety while you're underwater, so make sure to answer every question honestly.


First, ensure that your doctor signs the form. This is mandatory when you begin your training as an scuba diver. You will need a new medical release if you have any medical conditions. You should also take a photocopy your certification card.

Logbook

Before a diver can dive open water, he or she must complete a medical statement. Generally, this will be done before a certification course begins. A doctor must sign the release. An updated medical release is necessary for patients with changing medical conditions. A medical release is available through PADI. The medical release is equivalent to the PADI Release of Liability/Assumption of Risk/Non-agency Acknowledgement Form - General Training or the EU version.

The universal referral form differs from the one in a logbook. It will detail the number of open water dives the student has taken and the knowledge development sections they have completed. This document also includes a statement signed by the instructor verifying that the diver has completed the required dives. The student must then take this form to the instructor, who will certify him/her as a certified diver.

You must complete the course

The Universal Referral Program covers most well-respected scuba certification bodies. This allows a diver to complete both the theory and pool parts of the open-water course at his or her local dive centre, while also completing the open-water check-out dives at another agency. This can only be done in cooperation with a dive centre that is a member of a universal referral programme such as SSI (NAUI), NASDSS or YMCA.

PADI Open Water Diver courses require that the student passes the Knowledge Development and Open Water Dives sections. Before diving, the student must sign a medical release from their physician. To be certified, the student diver must take a photo. Digital images will be accepted.
